Elder Care File is operated by Moh Bami. It is not affiliated with the Centers for Medicare & Medicaid Services or with any state agency. Every survey, deficiency, penalty, rating and ownership record on this site is reproduced from the official publication, dated, and linked to its source. We do not rate, rank or recommend facilities.
Method
Federal records come from CMS Provider Data and are refreshed on CMS’s monthly cycle: the nursing-home provider file, survey summaries, health deficiencies, penalties, ownership, Payroll-Based Journal staffing and quality measures, plus the home-health and hospice provider files with their published quality measures. Every figure is loaded as the agency states it, and each section of a record page names its dataset and the date it was retrieved.
CMS publishes each nursing-home citation’s tag, official description and scope/severity letter, but not the surveyor’s written findings — those appear only in the Statement of Deficiencies (CMS-2567) for the survey, which we link on every line. State assisted-living records are being added state by state as each agency’s severity mapping is reviewed by a person; where a state publishes its inspection reports as PDFs we archive them unaltered.
Our only additions are a three-class reading aid beside the official severity code, topic labels from a fixed list, and links between records that share an owner. Chain groupings are CMS’s own; we never merge two differently-named entities without a person reviewing it. Individual owners are never merged across facilities, because two people can share a name.
Funding
Self-funded. Display advertising and a paid family report are planned. We never take referral or placement fees from any facility or operator, and no payment changes what a record shows or where it appears.
Corrections and privacy
Every page carries a report form. Corrections are checked against the official record; privacy reports are handled within 24 hours; removal is always free. No resident-level data is ever collected or published, and the site contains no protected health information.
This site is not a consumer reporting agency and its records may not be used to make decisions about employment, credit, insurance or housing. Records are for informational use.
